99.9% of the time, this is done by someone already under HIPAA rules due to their job or people who are hacking into databases.

If we all know about your prolapsed anus because someone told us, we can talk about it and it’s not a HIPAA violation. If your doctor tells us about it without your permission, that is a HIPAA violation on the part of your doctor. We can still discuss your prolapsed anus, without violating anything. How did you get a prolapsed anus?
