Have any titles been altered due to vacated wins? Ex.: divisions now have co-champions, etc. If so, what happens to them?

If I was Ole Miss' chancellor I would make it my life's mission to ensure that Ross Bjork never rises above being a janitor in Starkville for as long as I am breathing air on this planet. This may sound God awful (it is) but a janitor in the greater Starkville area is in the top 5% of income earners.

No. That's why the NCAA stopped forfeiting (see Kentucky's retroactive 1976 SEC title).

No, the NCAA doesn't do that anymore. For instance, there is no recognized national champion by the NCAA in 2004. USC just had their title stripped. Teams who lost to Ole Miss don't now receive credit for a win either, they just get to remove the loss from their record and Ole Miss their win. It's been a long time since the NCAA awarded forfeits and retroactive titles as part of sanctions.

USC wasn't stripped of the national title in 2004? I know they had to vacate all of their wins from 2005 and the last 2 games of 2004. I also know the BCS stripped them of their title, making it the first time a national title had ever been stripped. What am I missing here?

But does the NCAA still recognize them? They obviously never updated their website because it says BCS title next to their name in 2004 and that was obviously stripped. One would assume that given that their last two wins in 2004 were vacated by the NCAA, one being the national title game, that the NCAA no longer recognizes that title. The AP might, but they are not governed by the NCAA. I know we're getting into semantics here, but I'm genuinely curious as to what the NCAA's stance is. I guess it doesn't really change the overarching point, though. No one was retroactively awarded the BCS/Coaches title.
