Might be Germans but why can't he throw back to the pitcher without this horrible hitch?

He's got Tourette's, Lou Gehrig's disease and I think Multiple Scelrosis all in one I think. That's a liability for a catcher.

It's something about him being inaccurate if he throws it too quickly when he's in the stance. Weird and bothersome though.

Fwiw, I'm at the game, right where he's near. He nods Everytime, he's communicating with the pitcher.

Jarrod Saltalamacchia had the same issues when he was with the Rangers. They sent him back down to the minors to get it worked out. He would occasionally have "relapses". He's with Boston now, and he'll still occasionally just throw it away, he really has to take his time.

Our catcher in high school had a problem with something like this. For a while, he would throw the ball into the grass in front of the pitcher. Never did figure out why. Could burn people at 2nd, but couldn't get it back to the pitcher.

My theory is at some point before his collegiate career his threw it too quickly and dinged the hell out of one his pitchers. Maybe seriously hurt someone and it haunts him forever so now he has to hesitate.

It's called the "Yips". It's a mental block that some catchers get after they've played the position for so long. Throwing back to the pitcher is a common routine but if one thinks about it for too long it gets into the head and forces him to throw the ball short or throw it away.
Serious problem and it's sad because it can ruin a career.
