In simple terms of number of people needlessly killed, the answer is the Great Leap Forward.
In terms of sheer atrocity, the answer is the Khmer Rouge regime in Cambodia.

Nope. What happened here is the worst thing a group of humans has ever done to another group of humans. Both in terms of what was done and the fact it was intentional and meticulous.

Unit 731 is definitely up there. Honestly, between that, the Holocaust, the Khmer Rouge regime, the Hundred Flowers campaign and the Cultural Revolution in China, the Holodomor, the Armenian Genocide... the twentieth century had no shortage of truly awful and reprehensible acts perpetrated against humanity by itself. As to which one was the "worst," a good argument could be made for any of them.

The Japanese empire in general during wwii was probably worse than the nazis, but for some reason they get a pass when it comes to the two regimes at the time

Sending an entire generation of men into the meat grinder that was the Western Front of the First World War is probably as awful an act as any. Hard to even find the smallest justification for the way that war was waged.

I see a lot of holocaust answers. If that's the case then the worst thing mankind has ever done is allowing Germany to even exist after WWI. Should have nipped that in the bud. The fact that Germany is still a country is a major failure of mankind. Now they pretty much run the EU and make decisions for all of Europe.

Germany was allowed to exist after WWII because their academic elite were the best in the world. We would have had no space program without a former Nazi. They were also the most industrialized and well educated population in Europe. Second onlynto the US in their ability to mass produce. Without an intact Germany Europe would be a 3rd world entity today.

Don't forget Uncle Joe Stalin's collectivizing the Soviet Union's agriculture, an event that killed a huge amount of people, though its impossible to be sure just how many died as the records were destroyed. Estimates are between 6 and 13 million people starved to death as a result of the program, many in the Ukraine.
Which took some doing because the Ukraine has some of the richest farm land in the entire war.
Famines tend to be the result of either deliberate policy or sheer bungling (God help you if Britain was in charge of your country's agriculture). We even had our own plan, that we put into place after WW II, to starve our areas of occupied Germany, and were doing so right up until the Cold War started to heat up and we realized the West needed a strong West Germany.
