They have fairs in every city and dozens at any time. Yet these accidents are rare. More people die at Disney or Six flags and in these cases usually few injuries. I always liked fair rides.

The carnival industry is interesting. Insurance is expensive and requires proof that competent employees are being used. But, you look at a lot of carnies and think "no way." I was surprised to see the zipper crash because it is known as a dependable ride. I'd be interested if it was a newer model which would have more tech and likely less robustness. The newer zippers also have bigger passenger capacity. The 50's and 60's Sellner tilt-a-whirls are still going strong, the same with the Big Eli (Bridge Co.) ferris wheels and scramblers. I sometimes wonder if average passenger weight has created some issues.

In a way, carnival rides are amazingly sturdy because they are pieces of heavy machinery that is taken apart and reassembled on a regular basis with failures being rare. Conversely, we trust carnies to assemble them correctly.

I am one of those that has NEVER trusted carnivals or amusement park rides. Ever. I know places inspect and this and that but what if one pin or bolt comes loose. What if that clamp on the coaster breaks off. Most of these things are safe but I don't want to be, like these people, on that one that isn't.
