A close friend who worked for the coroner and in a funeral home told me this is a lot more common than people realize

only talks about fainting (syncope) in story

valsalva is just trying to exhale against a closed glottis (like trying to blow into a baboon that doesn't expand), not some mystifying condition that leads to death

In a healthy person the valsalva maneuver isn’t likely to be dangerous, but it does cause hemodynamic shifts, is associated with cardiac arrhythmias, and is linked to sudden death.

I had medical problems several (8) years back that resulted in me eventually getting a pacemaker. The problem was that occasionally my heart would simply stop beating, usually for just a few beats and then start back up (called syncope).

Then one day, it stopped for about 15 seconds (catastrophic syncope) and I passed out while driving down the street. Eventually they installed a pacemaker and all has been ok since. It starts my heart back up maybe 3 or 4 times a year now, and paces maybe 20% of the time.

I also have recently developed atrial flutter, which is controlled with drugs, but they have had to shock me to reset my heart beat rhythm twice.

When they were struggling to find out why my heart just stopped beating, my cardiologist asked me to really strain as if trying to have a bowel movement while they had me hooked to an EKG and had a crash cart in the hospital room.

The short version is I told my cardiologist it had been nice knowing him, told my wife I loved her, said good by and then strained down as hard as I could. Thankfully, my heart didn't stop (that time) and even my cardiologist's nurse seemed happy.

He explained that some folks pass out and die on the toilet. Yes it has happened. I'm sure it is a tough way for the family to have to explain to others how you died.
