Most physical pain you have had?

Jumped out of a boat in my teens as we were docking and broke both the bones in my arm. It looked like an S. I've had some other injuries but nothing came close to that, not even a dislocated knee.
Bonus: When my father was a teen he broke both his arms trying to ride his bike between to trees. Dummy.

Took a miniature replica of the Empire State Building, turned it upside down and stuck the spire down my urethra. Then I got one of those things that you shake and it snows inside and hammered it down my pee hole a few inches more. Jesus Christ... talk about painful!!

Waking up from my second back surgery in under a year. Honorable mention goes to gall bladder attack due to gallstones.

Not sure if this exactly fits but the worst pain I've ever felt in my life (and I've had plenty) was when I had a hole in my ear drum. I didn't know it was there and had gotten back from the pool. I'd had problems with ear infections since birth so I put Swimmer's Ear in to help get any water from the pool out. It was like liquid fire and it spread from my head to my entire body.
After that I put up with pain and blood coming out of my ear until the hole in my ear drum could be patched with surgery. To this day, I get panicky whenever a doctor examines my ears and whenever anyone or anything gets near them. The thought of using ear drops will make me nervous as hell.

I tore ACL playing basketball in the 11th grade. At first, it didn't really hurt, but after the shock wore off, it felt like there was something on fire inside of my knee. After surgery, it was just a numb, dull pain that was constant for a couple of weeks until the major swelling went down.

I had a kidney stone while in Disney World last year. Hurt like hell and I missed most of the vacation. Been through 2 knee surgeries, a vasectomy, all four wisdom teeth cut out and never took any meds for the pain. I was gulping them down last year.

Your guess is as good as mine. He definitely misjudged the width and I think he had both arms bowed out in the handlebars.
I have a great picture of him in his old style plaster cats with the arms held up from a piece that attached to the ribs.

Dislocated my shoulder twice and put it back myself both times. It hurt.

Oral surgery with no anesthetic.
Seriously, folks today have no idea how far we have come with painkillers in the last 50 years. Now they zap you with some knockout drug and you wake up in the recovery room and still doped with some happy juice so if you have pain, you can't feel it.

Almost getting my thump ripped off in 8th grade PE class. Guy and I were goofing off while playing soccer, and he got pissed at me for some reason, grabbed the soccer ball and was going to punt it at me. I put my hand out to block it and his foot caught my thumb and bent it completely back. Torn ligaments and broke 2 bones. It stunted the growth of my thump and now my right thumb is about 3/4" shorter than the left one.
I still get a little queasy thinking about that pain. It was intense until I got to the ER and they knocked me out for surgery. Like my whole arm was throbbing and felt like it was fire.

Hmmm, the blast effect of a bouncing Betty. Had a headache for months and a raging UTI from a catheter. Then followed by unfortunately catching a ricochet or ripping my pec off.

Coming out of jaw surgery after breaking it in 3 places, including a total break of the lower mandible. I started wearing my seatbelt after that.
