None of this seems to be a problem where universal healthcare has been implemented.

That’s not really true. Our quality of healthcare is so high largely because the competency of the people becoming doctors, nurses, etc. is higher than other countries.

Basic economics says that if you decrease the incentive to be a doctor, the quality of candidate entering med school will also decline.

ETA: a very likely consequence of universal healthcare is that your best and most experienced specialists move to an all-cash model outside of the insurance system in order to maintain their own lifestyles. Younger doctors won’t have that option very often, so we will struggle to backfill with similarly gifted new candidates.

Ding ding ding! We have a winner.

This is absolutely the correct answer. You think people are going to do 8 years of med school and 4 years of residency if a high salary isn't the reward for all of that work?
