In this instance you aren't leaving for a competitor in the same market, more pay, or to slight the current employer, it's just because you need to make a drastic change for the future of your personal health.
Also, the reason you have the new opportunity is all due to the experience you gained from your current employer, so quitting on them would sever a lot of ties to the people who helped you grow professionally.

Just be honest and direct...unbelievable opportunity came up...so thankful to work with y’all...blah, blah, blah...in person, not in a letter or email.

You're not quitting on them. You're doing what's best for you. If some people sever their connection with you over it, they're selfish assholes.
I'm sure in the majority of cases that won't happen.
I would write a succint letter, ask for a meeting and have your supervisor or whoever read it front of you and then have a discussion.
Maybe they try to keep you?
Either way you don't have to be ashamed of doing what's best for you, especially if its for your health primarily.
Everyone should understand.

If they are good people they won't stand in your way. I transferred to another department and my boss was sorry to see me go. I did a PITA job that no one else wanted to do and did it well. He told me he wouldn't stand in my way considering it was twice the increase in pay. Just talk to them.

Its business. Just put your resignation letter in and talk it over with your boss. Give them your two weeks notice and then you are out the door.

This is ridiculous. Companies can give 2 shits about employees. They want to make money and pay just enough to keep people from leaving. So why should you care?
