I've done a paper on torture back in college so I'll take a stab (hehe) at it.
First, take a small hammer and begin to shatter teeth one by one. This takes quite a while and gets progressively more painful without inducing shock. Then shatter fingers and toes one by one. The rack is also excellent because it is excruciatingly painful while also having no external blood loss (internal bleeding may occur if tightened too much due to tears in veins and arteries beneath the skin. Continue to break bones while avoiding compound fractures. Abrading the skin is more painful than flaying and also results in less blood loss which if you want to prolong torture and execution is important (obviously). The so called Pear of Anguish (although no first hand accounts of widespread use in torture exist) could then be inserted into whatever orifice the executioner desired. The pear is a mechanical contraption in a spheroid shape where a screw is turned at the end of it opening the "petals" wider destroying whatever bodily orifice into which it is inserted. It would break jaws (made even more incredibly excruciatingly painful by the broken teeth) or destroy the rectum. Adding salt to the abrasions and further abrading the skin might be the next step.
End it with pressing. Place the shattered remnant of a man under a metal plate and add weight until he is slowly crushed underneath.
Just to be clear, I'm not necessarily advocating this. This is just what I can come up with that would be the most painful, excruciating way to die that I could think of.
Force feeding pork before shattering the teeth would be a good psychological torture as well.

I'd keep him well fed and alive for years. I'd also send him periodic videos of his wife and sons living in America, learning English and openly campaigning against his life's work. I'd probably do some clockwork orange shite at some point to turn his opinions around and then have him totally screwed up mentally. Maybe like reek in GOT, but I dunno. It would get creative.

But it would involved rats and pigs and amputating digits and limbs over time, stretching it out through transfusions and drugs ... giving him time to recover before taking something else ... it would be ugly for him. I'd get a lot of information from him before the final disemboweling and feeding to the pigs. Of course, by that time, he wouldn't have anything but his head and trunk remaining ... and the rats and pigs would have developed a taste for him by then.
I would leave him at least one eye and one ear as well ... right up until his end.
