Oh, there's enough blame for everyone involved.

What's kind of funny (or sad really) is that when Notre Dame was found to be at fault for the death by OSHA, they did the blame shifting game. Nobody was identified as being the one who made the decision to use the lifts. They pointed at the ND coaches as being the ones who decided "against the athletic department recommendations" to have practice.

The coaches said that they decided to have practice but the decision to use the lifts was someone else's call.

So effectively the university got a small fine and nobody was responsible for the kid dying due to the blame shifting game. The AD blamed the coaches. The coaches blamed the AD. Nobody got in trouble.

They all got incredibly lucky that the family didn't want to sue everyone involved. I imagine those depositions would have been very interesting regarding one current coonass coach.
