We hear a lot about title 9 violations. Has a school ever been punished because of it?

I would like to see what schools have been punished.
You here about title 9 issues but nothing ever happens.

Because most are allegations with no proof.

Punished by the NCAA? No, I don't believe so. I think the reason why is until recent changes to the bylaws, irrc the NCAA didn't really have any authority to punish schools for Title IX violations. I believe there have been some modifications to the bylaws to include certain Title IX compliance rules. Before, without any bylaws addressing those types of issues, the NCAA realy didn't have the ability to say schools violated bylaws that didn't exist.

Title 9 falls under federal funding. SO what tends to happen is if your school gets hit with a wave of title 9 problems they correct things because otherwise they lose federal dollars.

And you likely wont. What I have been telling posters on here is that Title IX is a Federal Regulation, not a NCAA rule. Sports is only a part of the overall regulation.
Enforcement is through the courts so I would imagine institutions and plaintiffs often settle privately and many sources of info are gagged for pending or settled litigation. A lot of the LSU info came out as FOIA requests or press releases due progress reporting.
