At the start he played all the times the Marlins have hit Acuña, and they're all the exact same thing. First pitch of the AB, catcher sets up for an up and in fastball, they hit him in the upper arm/back.

I saw a tweet today that tells you all you need to know. Since 2010 Acuña is the only player to have been hit on the first pitch of a game 3+ times. The Marlins have been the team that hit him every one of those times. Honestly I think Lopez did do it on purpose. Alfaro set up for them to buzz the tower. His reaction was because he wanted to plant it on his ribs and put it somewhere that could have seriously hurt him.
