I think it is reasonable to assume that he played sports from junior high through high school which means there were plenty of chances during yearly physicals to test for the trait. This means there were several physicians who failed to diagnose the condition long before he walked on to Ole Miss' campus, and this is completely ignoring the family who has to know the diseases tendency to follow the genes in families...

It is unfortunate, but there isn't a villain in this story, several people failed to protect the kid, sadly, including his own family...
