There were several official investigations into the incident, including one by OSHA.
None implicated Brian Kelly in any wrongdoing. If he had been guilty of something, it would have been fairly easy for Notre Dame to simply fire him. That would have satisfied the PR disaster.
But some posters here are fairly desperate to pretend he was responsible.
