This is literally all you need. Just crop it to fit the sig image. The additional text just clutters it.
If you edit it at all, just copy and paste to make the score text at the bottom larger.

Just take the image from above, and cut, and expand the scorecard at the bottom to fill up the entire width of the bottom of the pic. It's perfect.
