I signed one too and was 18. I don't remember my parents signing it, but they could have. It has been a while and things have changed. I would assume the signature of just one Guardian would be sufficient, but maybe not if they have joint legal custody.

Even if his dad doesn't want to sign he can just wait till he turns 18 and do it himself.
