I hold it upright aka downward and rock it back and forth holding it with my fingers by the loop at the top. Or you can hold it at the side and rock it for a more muted sound. It has.. had a nice booming sound. In the mean time I've made a temporary "clapper" with some stainless steel nut and bolts but it doesn't stay good, comes undone, certainly doesn't sound as good as it did.
That worn out tape is proof that it takes a toll on the knuckles. A buddy of mine was talking about fashioning a heavy duty handle with a bicycle grip on to it.

Find a leather-smith to make a "belt" through the loop. Saddle leather should be stiff enough to give you a 'handle' to ring it properly...Please don't cut that bell, it looks awesome as-is. For clapper replacement, I used a couple of these chain repair links, and they work fine.
