I would think you would have to partially inflate the balloon first. I've never personally made a piss balloon, so I'm interested to know how it is done as well.

Not that I would know (but you seem to infer some experience) but wouldn't a smaller dick be easier to fit a balloon around? I feel like piss balloon is more of a misnomer. Piss trash bag just doesn't roll off the tongue.
