I nearly ran over my daughter with a suburban. It was 5 or so years ago, but it still makes me sweat just thinking about how close I came to killing my own child.

Why did you have one of the 50+ people in the house take you to the hospital?

i think that might be most embarassing moment qualifier... but that's not actually in the top 5 sadly.

Probably the time we were throwing water balloons at cars on a Friday night campout when we were boys. We lived in a subdivision right below a main highway and around three o'clock on a Saturday morning a camaro came down the road balls to the wall.
I timed a throw just right and put one in the passenger window and clocked the driver in the side of the head. He did about half a dozen 360's and then he and some of his buddies came back after us.
I dived into a huge brier patch and remember them cursing and saying what they would do too us if they could get us.
I still remember that bearded dude with the big red mark on the side of his face cursing and flaying that tire iron around in the bushes. The man threw a cigarette about a foot away from my hiding spot and I don't doubt for a moment would have at the least beaten me senseless or, ended my time on earth right then.
But frick that fruck he got a wet head that night.
footnote # This was the last water balloon I have ever thrown.

I've been lost in deep forest several times at night and I immediately go into survival mode. If you can make it thru the night, then you're good to go.

And? If your life was in danger I'm sure they'd understand
My moment had to be in my friend's SUV during a massive rain storm. He took a sharp corner and spun out going over the sidewalk taking out a tree and nearly flipping it into a gas station parking lot.

I was approached by a middle-aged pervert in a vehicle in what could have been an abduction situation as a 12 year old on a barely traveled road while walking in from deer hunting. I rejected his advance and thank god he didn't try to escalate things and drove on. I had unloaded and cased my rifle, so he could have easily grabbed me.
When I told my dad he immediately went out looking for the vehicle to kill the mo-fo and probably would have.
I was actually more scared later, than at the time. Very, very fortunate.

Last spring. Myself, my girlfriend, kid I kind of knew, and his girl where at my buddys house. We were watching the NBA playoffs and they were smoking. Me and my girl weren't. His girl gets up and goes to the bathroom and there were 3 loud bangs. Then she walked out. I was sitting next to the door so I could see her face when she came out. It was just colorless and empty. She took 3 steps and barely said "I'm so hig.." anf then she just dropped. Now my buddy had this box on the floor where she dropped and her head missed it by less than an inch. So she's on the floor, eyes shut, my buddy goes "I'm gonna go get water" and leaves the room. This guy jumps up and starts shouting "BABY, BABY BABY, etc" and myself and my girl go to help her. She just starts convulsing and her eyes start rolling to the back of her head. I was getting ready to call 911 and start CPR but then she just stops and wakes up. She was kind of out of it but she was able to get to the bathroom where she was there for at least an hour with her boyfriend. Then they both came out and left

I've got a lot. The number of times I've tried to kill tried to kill myself (figuratively) is not something I'm proud of.
But this thread reminded of the most recent. I'm hiking in the Mark Twain national forest a couple of years ago. As usual I've left too late and gone too far and now it is dusk and I'm still a 2-3 miles from the car. Then it clouds up and gets too dark too see. And starts to rain. Temperature is in the low 40's probably heading to the high 30's and I can't see my hand in front of my face. I've got enough rain gear to be ok but I'm looking at a cold, wet miserable night in the open. So I decide to orienteer my way back. I know that the car is to the south east and there is a gravel road that I have to cross if I head that way.
So I set out and slip and fall and cross creeks and get wet and muddy and cold in the rain in the woods. Eventually I get to this tree line with a clearing beyond it and I can see a light off in the distance that I think is the light by the old church where I parked. I was expecting all these landmarks at about this time so I'm relieve, but I haven't crossed the gravel road which seems odd. Oh well only going to find out going forword. So I step to the edge of the trees and stop. Something doesn't seem right. The light is too far away, and I can't see anything in front of me at all. I mean it's very dark, all overcast, no stars anyway, but in just seems to be blacker in front of me.
For some reason (and I don't know the reason) I stop and stand there trying to decide what to do. Never once does it cross my mind that I'm in danger I'm just trying to decide if I'm lost. I just stand there for several minutes thinking about it and finally I decide I don't know where I am and going forward could just be getting farther away. I decide to turn around and circle in the dark looking for an old logging road I know is there somewhere. I stumble across it fairly quickly (finally a lucky break) and can follow it out. I'm kinda pissed because it adds another couple of miles to the hike and it's like 10 o'clock now but at least I know where I am. I break over a couple of branches to mark where I am and point where I came from because I'm really curious how lost I was. An hour later I'm at the car and go home.
The next weekend I decide to go see if I can find where I was. Good weather, bright sunshine, warm, middle of the day. I hike a couple of miles to the logging road and then go down it looking for the branches. Finally I spot them, pretty obvious and head in the direction they where pointing. Fairly quickly I come up on the tree line with the clearing. I can see an old farmhouse in the far distance. I step through the trees and in front of me is a sheer riverbluff with a 80-100' drop to rocks below. My toes are practically hanging over the edge.

I was the passenger in my college roommate's car, on the way home from a spring break trip. It was late and we were driving on a desolate road back home. It was a slow drizzle of rain, the steam of the hot pavement that reduced the visibility even further on this moonless night.
After a couple of hours of driving, we pull into a small town that seemed fairly abandoned. There was one restaurant open, Arby's, so we hit the drive thru. In typical small-town fashion, our food takes forever. They finally hand our bags of food and drinks in and we're on our way again. We slip out of the town and back into the rural darkness as I'm digging thru the bags of food. I open my roast beef to put on some Arby's sauce and as I am finishing a passing car light illuminates the interior of our car just enough for me to see it. I put mother-fricking Horsey Sauce on my roast beef instead!!!! I nearly ate it too. Still makes me shiver.
This was some seriously scary shite

Watching my little boy go into crib death at the hospital and knowing there was not a effin thing I could do about it.
Watching Mackenzie Phillips go into cardiac arrest 10 yards away from me and then hearing the shock thing not register a heart beat first couple times as stadium was spooky silent.
