It's only been used when teams where caught systematically cheating, and then continued to do so.
SMU's boosters chose to honor their word and continue paying players as ageed after sanctions were handed down for paying those players.
OM better hope their recruiting strategy changed after being caught the first time.

Thank you for responding but it still does not answer my original question. I would guess that the FCS has never seen the "Death Penalty". I would think that they have established rules that are thoroughly enforced. Perhaps.
