I think it was mainly player execution.
Two main things were the issue IMO.
Kennedy was hurt for the middle of the year and he seemed to have the most success going over the middle.
When he went down Kenny apparently lost the will to throw his wobblers down the center b/c nobody else was getting as open as MK.
Once MK got back and Kyle Allen took over the middle field passes started to return.
