Cold Urine (smuggled in) is considered a diluted sample and a fail.
Some of the older masking agents, or wive's tails on how to mask, also comes back as a diluted sample and a fail.

who the frick sneaks in anothers urine but isn't bright enough to get it up to temp?
it is considered a fail but not by dilution.
