High risk of stroke is very much related to obesity.

ETA: One's risk for any sort of cardiovascular problem can be greatly exacerbated by being overweight, eating shitty food, and smoking. This why states with the highest stroke risks (as well as highest risks of heart attacks) are those with the highest blood pressure, obesity, smoking, etc. It's all connected.

ETA2: And of course poverty and intelligence are correlated to these issues as well. The poor and less educated tend to be less healthy than the wealthier and educated. It's a chicken vs. the egg debate, for sure, but the correlation is most definitely there.
