these things happen. It's a shame but it's a fact

I can't stand this comment. I hear it all the time and it makes my skin crawl. There is always a safe way. Especially when it comes to preventing fatalities. Somewhere there was a failure that could have been prevented. It did not just "happen."

I didn't say it just happened. I said these things happen and that is fact. I don't know what exactly transpired for this accident to occur but I never said this or any accident couldn't be prevented. Don't twist what I said.

It is possible in construction for everyone to be doing everything right, and yet a fatality still occur. Hell, it's possible for that to happen while your driving down the road. Joe is driving down the road, a blowout occurs on his front tire due to road debris that he can't see, his car shifts to the left, where it hits some loose gravel and spins out, sending him right into the semi coming from the other direction.

I have no idea what happened in this case. None at all. But I do know that terrible accidents can sometimes happen even when everyone does everything right.
