It seems like the city is to blame here. With that many accidents, they should address the elevation of the road. If it is not doable they should clearly prevent box trucks from taking that route. I am sure once debris flies off and causes a death directly or indirectly though they will address it.

i'm pretty sure the blame lies on the drivers who don't pay attention to the height of the bridge signs, but that may be just me.

Truck drivers are required to pay attention to clearance signs. They get told over and over I'm sure. The fault clearly lies with the driver.
