Well the NFL is interviewing people to get the full story.
The story that is out and can kinda be pieced together is that Foster was waiting for the "pre-exam." (I saw someone on twitter (I think Bryan Broaddus) say that it can be a long day waiting for an MRI - so take that as you will).
It was taking quite awhile so Foster questioned a hospital worker. Supposedly he used the ever-popular "Do you know who I am?" line, which didn't impress the hospital worker who said that Foster would wait like everyone else.
Then Foster said he'd lay his hands on the worker, and the worker told him to "Do it."
This led to a face to face heated argument between the two.
