could likely be a failure to pay a traffic ticket, then failing to appear in court or pay the fine by the court date. it's not as big a deal as it sounds, although it is dumb and irresponsible

no it's pretty simple. you don't show up for court, you have a bench warrant issued or, in some cases, your license suspended, depending on the offense and state you're in. this is a matter of personal responsibility and something that's easily avoidable.

no it's not. i've worked in state court. if you can't afford the fine, i've seen up to 5 continuances granted. judges also frequently will let people do community service instead of pay a fine. all you have to do is show up to court when you're supposed to show up. it's not a difficult concept, and it isn't just the government being assholes.

No consequence? Any entity that takes on debts from consumers does just fine. Besides collections, you could double their points on their license if they don't pay by a certain date. There are a multitude of alternatives to issuing an arrest warrant for a petty and insignificant occurrence.

Well great on that judge, but that's not the usual occurrence and you know it. I'm not arguing it's not an easy situation to rectify, I'm just stating the consequences shouldn't be arrest over an something so insignificant. You can be stern and consequential without arresting people.
