The package insert is included in the vial box. Every one of them. No exceptions.



In the case of non-prefilled syringes, there is no package insert associated with "the shot". That is true of the CV19 vax, or penicillin, or fentanyl, or any medication drawn from a vial.

So truth be told, you have never seen the "package insert". And just FYI, your suppositions about my motivations for pointing that fact out leave you looking silly.
