Dr. Charles "Chuck" Smith is an Assistant Professor in the Department of Family & Preventive Medicine and Medical Director of the UAMS Neighborhood Clinic at Rahling Road.

I'm not sure about a doctor but you should be able to find some PCP there are lots of those types of people in the area.

Not particular as long as its within a 45 minute drive.
ETA: I work and live downtown. Anywhere within Little ROck or within 45 minutes of would be nice. I would even consider a bit further if someone gave a glowing recommendation.

not sure about PCP or Preventative care...
